For CENOMAR dapat kayo magrequest sa SM since need signature niyo sa form as well as birth certificate. Pero kapag release na ng documents okay lang if my representative as long as my authorization and one valid ID.

For those having representatives to get their birth certificates at SM, make sure to give an authorization letter and your ID.

Requesting and picking up the birth certificate can be done by an immediate relative, but if you're just the fiancee, you still need an authorization letter and ID.

Let me clarify that for CENOMAR, I requested for mine and fiancé's, wala naman hininging ID or what. His presence is not required din. I think you're only referring to Birth Certificate.


Upon claiming, the requester should present an ID.


By the way, applied our CENOMAR last Monday lang, Sept. 21.

Come to think of it parang anyone who has your information can request for your Cenomar kasi pag nagrequest ka online as long as you have the information. Pag claim lang or deliver mo need magpresent ng ID but some who delivers it are not even strict. (for ordering online)

ang alam ko din kahit sino pwede magrequest and magclaim ng cenomar as long as you have the info. unlike for birth certificate, sa likod ng form kailangan isulat name ng nagrequest. at dapat immediate relative na may authorization letter and id pag nagclaim

yes sis yannuh, double the price nga lang talaga if online. Unlike sa SM, +20 lang. Then hassle free na din coz parang kahit saan may SM. In my case naman, since I work in Ortigas area, walking distance lang ang Megamall

True sis yannuh hassle free nga. ni request lang din namin cenomar online or any Birthcertificate na kailangan namin pero correct ka sis twinxz and shygirlme DOUBLE the price sa mahal!

we ordered our [textspeak!] and CENOMAR online, got delivered in less than a week. sayang didn't know we can request them sa SM at a cheaper price, may SM pa naman in our condo building.

talaga sis? kasi if direct sa NSO 10-15 business days, same with SM. Hmm. Bakit kaya mas maaga if online? Hmm.

Paano ba proseso nito? Taga Pasig kami at Live-in na kami ng 8 years,may 1 anak.. This Dec namin plan magpakasal, This week ang dating ng cenomar from e-census. Dito ba sa Pasig paano proseso? Mag attend pb kami family planning kahit matagal ng magkasama? Ilang oras yung family planning seminar?  thanks!!
