i dont think magagamit mo kasi diba mga anak na 18yrs old and below ang qualified dependents?
workng ka ba? pwede ka rin mag voluntary pay. apply ka na lang tapos bayaran mo na lang at least 9months before your due date

ugh I think considered ka pang dependent basta hindi ka mag 21 til your due date... and as long as youre not married. pero [textspeak!] baby mo, hindi na covered ng Philhealth ng papa mo. as usual, sabi nga nila, you need to get a copy of your papa's MDR sa Philhealth office, nakalagay don [textspeak!] eligibility date and dependents nya. :D

PhilHealth card packs purchase discountsBy JENNY F. MANONGDOSeptember 5, 2011, 4:46pmMANILA, Philippines — The Philippine Health Insurance Corporation (PhilHealth) launched Monday in Pasay City its new member identification (ID) card that enables the bearer to claim discounts on purchases from participating commercial establishments nationwide.

“We launched this new ID card because we are employing another approach to expand our membership,” PhilHealth president and CEO Rey B. Aquino. “By introducing this innovative pull strategy to our membership registration efforts, we are optimistic that we will be able to reach our fellow Filipinos who are not yet covered.”

The new card may be bought at SM outlets or Bayad Center outlets for a minimal fee. The card will bear the member’s permanent PhilHealth Identification Number (PIN), while the magnetic strip allows merchants to verify whether the cardholder has sufficient premium contributions to Philhealth before granting discounts or privileges.

A member who wishes to avail of the new ID card in SM outlets will have to pay P360 which is broken down as follows: P300 for one quarter premium, P50 for registration and card and P10 collection fee. The Bayad Center outlet will charge P358 for the same card.

ID card applicants need to present a valid ID; copies of supporting documents for qualified dependents the applicant wishes to declare such as an original photocopy of birth certificate; and payment for the premium.

The following establishments are partnered with Philhealth in giving discounts to ID holders: Watsons Personal Care stores Phils. Inc: 15 percent discount for unbranded generic medicine products, both over- the-counter drugs and prescription medicines, 10 percent discount on Watsons branded cosmetics, 10 percent discount on compliance packs; 5 percent discount on medical devices (no minimum purchase required), 50 to 60 percent discount on Qualifirst products, P380 flu vaccine (includes administration); Fresenius/ FMC Renal Care Corp: reduced price of P4,000 from P6,700 for hemodialysis (in 18 outlets in Luzon and the Visayas); PQ Health Shield: 20 percent discount on vaccines; Rose Pharmacy Inc.: 15 percent discount for unbranded generic medicines, both over- the-counter drugs and prescription medicines; 5 percent discount for medical supplies and devices; 50 to 60 percent discount on Qualifirst branded generic drugs and medicines; South Star Drugstore: 15 percent discount for unbranded generic medicines; 5 percent discount for medical supplies and devices; 50 to 60 percent discount on Qualifirst branded generic medicines; Vivian Sarabia Optical corp.: Free eye exam and 20 percent discount for lenses and frames (available at SM city-North EDSA, Cubao, Megamall and mall of Asia); J&W Drug Testing, Inc.: 10 percent discount on drug- testing services.

With this new approach, Aquino said they are primarily targeting the non-members who have the capacity to pay the premium contributions.

^ sis philhealth is a type of insurance, kapag hindi mo nagamit w/in the year, forfeited na yung contributions mo. sa pagkakaintindi ko sa discount card is it's not a replacement for old Philhealth ID, it's just an additional card for medicines. so parang kapag may discount card ka from philhealth maavail mo yung discounts from all their affiliated establishments... yun yung pagkakaintindi ko sa inquiry ko kahapon, kasi i'm from HRdepartment, then nabanggit ng messenger namin ito sa akin so immediately inquire about this para makapag-avail din lahat employees namin...

(im only 20 yrs old) thank you po kng sino mkakasagot. :)

Hi sis, you cannot use your father's philhealth kung dependent ka niya and ang case mo is manganganak ka. I THINK. Kasi parang HMO yan na pagdependent ka ng isa tao ang then ang dependent ay nanganak, wala use ang health card. pero go through the website din :)

mga sis, question lang po regarding philhealth

kasi nakuha ko na un certificate of contributions, MDR at philhealth claim form 1 ko from HR...
after I got married kasi I had my details changed to my married name sa lahat ng government forms including philhealth...dun sa certificate of contributions ko, married name ko ang nakasulat, pero dun sa MDR ko, maiden name ko pa...

do I still have to go to a local Philhealth office to verify na na-change na un last name ko?? kasi magkaiba un record eh
