Hi Doc!

just want to ask po pag nawala na ba ang kulani sa likod ng ear possible na wala na siya PC? kasi sa case ng anak ko as i mentioned earlier na diagnosed siya na may PC kasi may kulani siya and nakita sa chest x ray. hindi namin tinuloy ang gamot kasi iba naging effect ng gamot sa kanya.

we told her pedia about that and he said na choice namin mag asawa iyon and walang sisihan.. she never had cough and colds. hindi din sinisipon..now she's 2 and ok naman. wala naman na ako nakakapa na kulani, may weight gain naman siya pero hindi siya mukhang mataba.

^ Hindi clear indication na magaling na ang PC ang pagliit ng lymph nodes sa back of the ears kasi what if the enlargment of the lymph nodes there is due to some other infection? Since you opted not to treat and hindi macocompare ang before and after na clinical activities, in my opinion, the best way to find out if talagang nawala ang PC is to have a repeat chest x-ray done and have the 2 x-rays compared to check if there was any change.

@erl:
Hindi nakakahawa ang smoker's cough per se, since it is not an infection, but, of course, ang daming bad effects ng passive smoking and it can also induce cough sa mga na-expose dito.
Nahanap ba niyo ang source of your child's PC?

My son was diagnosed of primary complex. He is 20 months.

We are 7 adults in the family na may close contact to them, pero lahat kami clear sa xray. Do we need to have a repeat xray?

@erl:
TB ba or COPD ang sakit ni Lolo? Hindi kasi nakakahawa ang COPD, unless meron siyang COPD and TB at the same time.
Aside from Lolo, meron bang iba pang nagpa-xray sa household niyo?

@drex:
Pyrazinamide lang ba ang meds niya? Wala bang kasama na Isoniazid and Rifampicin yung pyrazinamide?
Kasi the usual treatment for PC consists of the 3 meds: Isoniazid, Rifampicin and Pyrazinamide. After 2 months of treatment, we stop the pyrazinamide pero continue pa rin ang Isoniazid and Rifampicin to complete at least 6 months of treatment.
We don't usually do x-rays after 2 months of treatment kasi matagal mawala ang x-ray findings. Hindi rin SOP na mag-xray at 6 months. Stopping treatment at 6 months would depend on clinical improvement.

@jhenbreak:
Aside from chest x-ray, meron bang nagpaPPD sa inyong 7 to determine exposure? How about AFB smear (examination of phlegm to determine activity of TB)?
Since 2 kids ang nagkaroon sa household, then most likely kung sino ang pinakamadalas nilang kasama ang source. Para magkaTB, usually repeated exposure yun over a long period of time.

@gheb:
As I said in my earlier post, hindi mawawala ang kulani since it is part of the lymphatic system and lahat tayo meron kulani, except na lang ang patients na may problem sa immune system.
As to kung kailan liliit or hindi na makakapa, it would depend on the cause of the paglaki ng kulani. Usually kung bacterial infection at nagtreat na ng antibiotics, after a few weeks, liliit na ito or babalik sa original size. Sa TB, it takes months for it to go back to its original size.

@erl:
If COPD lang ang findings sa father in law mo, then hindi ka nakakasiguro na siya ang source ng PC ng anak mo.
The first thing na dapat na ginawa nung nalaman na may PC ang anak mo is to find the source so that means dapat nagpa-xray lahat ng kasama sa bahay kahit walang nararamdaman.

@drex:
You're welcome.
2 months lang talaga ang PZA. Isoniazid and Rifampicin should be continued to complete at least 6 months of treatment.

Hi po, newbie here. Just want to have an idea if how much are the medications for PC. My son kc who will be 15 months next month ay nag-positive sa skin test nya yesterday. He has undergone chest x-ray kanina pero bukas pa namin makukuha result.

@jedijeah:
Did you bring your child for follow up during the time he was undergoing treatment for PC?
Sa follow up kasi ma-aassess if there is improvement in the clinical findings like weight gain, increase in appetite, decrease in size of lymph nodes, improvement in well being, etc.
Your doctor will be the one to say if there was clinical improvement and treatment can be stopped na.

@Doc Lelet just a clarification, the purpose of the BCG vaccine is to prevent the TB bacteria/virus from spreading to different organs of the body right? So why are you saying opposite?

@kermy:
When did I say otherwise? Would you mind providing the link so I can post a clarification?
As I mentioned in the PC thread 1 page 29, BCG protects against extra-pulmonary TB, which means it protects against spread of the infection to other areas of the body such as the liver, bones, meninges, etc.
