Anyone from Q.C. who already got a solo parent ID.  Where sya sa City Hall naa-avail and what are the requirements?  mabilis ba or you need several days to have it processed? Pa-share naman po. TIA!

^ hi sis! im from qc and i already got my solo parent ID last may. you have to go sa Social Welfare department. nasa may bandang likod na area sya sa loob ng QC Hall compound. i was asked to bring my son's birth certificate, and barangay certification to prove that im really from qc. tapos pagdating mo dun, papa-fill-up ka na lang ng form/affidavit na single parent ka talaga.

^ sis, bale yung sa barangay certification lang ako gumastos. close to 200 ata ang binayad ko just to get the certificate. pero yung sa mismong solo parent ID, parang wala akong ginastos.

^ mabilis lang naman sis.. i was able to get the ID 2-3 days after i have applied for it sa DSWD sa city hall. bale, kung saan mo pinasa yung requirements mo, dun mo rin kukunin. pwede mo rin ipakuha sa iba, kailangan lang ng authorization letter at ID mo.

jan sa solo parent idea, paano kung kasal kayo pero hiwalay na nga?

^ ang alam ko sis, you have to show some proof na hiwalay na talaga kayo legally, or na-abandon na kayo nung guy, and ikaw yung mag-isa na nagsusupport sa kids. though i'm not sure anong document ang pwede or dapat mo ipresent to prove that you're the only parent supporting the children.

^Sis, may proof of income requirements ba?  Right now kasi I'm in between jobs and just helping out sa business ng brother ko so I don't have any certificate of employment to present.

I'm planning to go sa QC City Hall this week.

^ in my case sis, hindi naman ako hiningan ng proof of income. pero dun sa form na pina-fill-up-an, may item dun asking the solo parent kung ano ang occupation mo. mabilis lang yung pag-process nila ng ID. hindi aabutin ng isang linggo.

^thaks so much sis!  I already applied for the solo parent ID pero sabi 2 wks daw wait ko before I call to check if ready na ID ko.

Another question mga sis:  we have a passport appointment on January 20. What documents should I bring for my daugther?  She's illegitimate and carries my surname.  No father on [textspeak!] coz he has always been unavailable.  I read in one forum that I may need an affidavit of ilegitimacy.  Ano pa po ba?

^ sis, parehas pala tayo ng case.. my son uses my surname and no father's name written on his birth certificate. when i applied for his passport, i just brought his NSO-certified birth certificate and my own birth certificate (to show how we are related). and as an additional supporting document, i bought his baptismal certificate. hindi naman ako hiningan ng affidavit of illegitimacy. if you have a friend or relative who's working in a travel agency, pwede ka mag-ask.

jan sa solo parent idea, paano kung kasal kayo pero hiwalay na nga?

kung hiwalay kayo (pero hindi annulled/legally separated) gawa ka na lang ng affidavit na ganun ang situation mo, and that afidavit should state that 1) hiwalay kayo ni hubby; 2)  may x number kayong anak ni hubby; 3) ikaw ang sole provider sa mga anak nyo. ipanotaryo mo, i think your neighborhood notary should be familiar with such a doc. then yung ang isubmit mo, to get the solo parent ID

^ sis, my son's birth certificate doesn't have a middle name. hindi ko rin kasi nilagay ang name ng dad nya dun. as in name ko lang ang parent sa birth certificate. so far naman, hindi naman ako nagkaka-problem when it comes to other documents like his passport and school records. though there are times, i have to remind their registrar and teacher na sadyang walang middle name ang anak ko.
