ask ko lang paano malaman ang philhealth number ko? at my last work kasi sila nagasikaso ng philhealth ko. posible ba makuha yun online lang?

O mas mabuti, tawag ka sa Philhealth o kaya pumunta ka na sa office nila, para makakuha ka na rin ng Philhealth card.

usually its the SSS number pero pagnaayos mo na yung member amendment mo, bibigyan ka na nila ng number talaga

^ correct sis.  SSS# muna yun, then once na kumuha ka ng MDR mo o member data record, may bibigay sa iyo na card and andun ang Philhealth number mo.

yan kase ang nangyari saken nung Monday, kase kelangan ko yung MDR ko for hospital.  sa mismong office ko nila kinuha, Lucena City branch.

Mabuti na rin makuha na yung Philhealth number mismo.  Kasi nangyari sa amin dati ng friends ko, sabi sa amin yung SSS number nga daw yung gamit muna.  Tapos na-confine sa hospital yung isang friend ko, saka lang namin nalaman na hindi pala nire-remit yung Philhealth contributions namin (actually, lahat pala ng dinededuct nila sa amin hindi nire-remit nung company, buti na lang nalaman namin).  Kaya mabuti nang alam mo yung Philhealth number mo para sure mo na ni-register ka talaga nila.  Saka kuha ka na rin ng member data record na sinabi ni Pretty_ava para ready na kung kailangan.

may sss na ako pero wala pa akong ID bale sa yellow paper lang sya.. yun na ba yung member data record? sorry super ignorante ko.. plano ko kasi magvoluntary member..

sis, ang MDR ay makukuha from Philhealth offices mismo.. print-out yun ng Philhealth member's details..

last Holy Week, my son was confined.. para ma-claim iyung benefits from Philhealth, kelangan magbigay kame ng MDR sa hospital.. feeling ko SOP na rin ito sa lahat ng hospitals..

buti na lang malapit lang sa Capitol iyung Philhealth QC branch kaya madali lang namin nakuha...

wala naman hassle sa pagkuha ng MDR tsaka ng philhealth ID.. na naka-indicate na din dun ang Philhealth # mo.. pipila ka lang saglit and mag fill out ng form then after few minutes, tatawagin ka na ng attendant to claim the MDR and Philhealth ID.

noong bago ako manganak, i emailed philhealth. nag-reply naman sila agad after 3 days. nagtry din ako magtext pero sira naman ata ang service.

You can also try calling PhilHealth and asking them for it.  Dati hiningi lang sa akin yung SSS number ko over the phone, tapos binigay na niya yung PhilHealth number ko.  Naisip ko lang na siguro kung identity thief ako, ang dali lang makakuha ng info pala from government agencies.  Pero thankful na din ako that time kasi it saved me the hassle of finding my lost PhilHealth ID.

make sure to keep the receipts when paying. mdr at contribution/payment ang kailangan when claiming or asking for deductions sa hospitals.

Ako rin naguguluhan mga sis. My husband needed my philhealth number kasi he's filling up his M1a form, ang binigay dito sa ofc namin is sss # ko lang. I texted philhealth and they replied na wala akong existing philhealth number.

Does it mean na hindi na re remit ng company ang contributions ko? I've been employed here for more than a year now kasi.

^ maybe you havent filled up the M1a form (application form for employed sector). Usually HR ninyo magdidistribute ng mga forms sa mga employees. Yan yung magiging problema sa remittance ninyo ng mga payments, pero probably SSS number ginagamit para sa posting ng payments ninyo. Pero sis, magfill up ka na ng form para hindi ka mahirapan in future. Tapos yung mga MDRs (Member Data Record) kailangan itago ninyo yon kasi mandatory na sa hospital na titingnan yon/aattach sa claim form.

question lang po.. possible pa rin ba may makuha or mareimburse sa philhealth if resigned employee nako? i resigned last may pero mgagamit ko pa kaya now?

tapos, pedeng dalin mo na lang sa philhealth office, ganun kasi ginawa ko nung in-enroll ko [textspeak!] father ko, para my pirma na nya pagdating ko sa philhealth office.
