help naman po.
first job ko po kasi, isa po sa requirements ko, SSS..walang Philhealth,TIN etc etc. meaning po ba, yung employer ko na ang mag aasikaso ng mga ito? or ako pa rin ang kukuha ng # ko sa philhealth at TIN?

then sa dependents po, need ng TIN,Philhealth or any documents indicating numbers.e yung dalawang kapatid ko po student pa lang so wala pa TIN.wala din Philhealth and Dad at mga kapatid ko. paano po kaya yun? kailangan po ba mag apply din sila ng Philhealth?

^ hi sis. tell your present employer na wala ka pang existing philhealth and tin # (since first job mo ito).

for Philhealth:
>HR will give you a form (M1a if im not mistaken). Filled this out and pass this to your HR. This form kasi yung kailangan nila na fill-up an mo (since wala ka pa philhealth #) para maireport ka nila sa phic together with ER2 (members data report). then phic n yung magbibigay sa yo ng philhealth number mo..

for TIN:
>same procedure lang din sis, fill out a 1902 BIR form (application for registration bla...bla...bla... ) then pass this on to your HR dept.

don't be afraid to ask you HR dept. Its your benefits sis. 

One more thing: it doesn't necessarily mean na pag wala ka pa ng mga numbers na yan, your employer doesn't remit yung mga kinakaltas sa salary natin. SSS# is enough to report your remittance sa philhealth and pag-ibig. Yung sa BIR naman, year-end yung reports ng remittance nyan..although monthly din nagreremit, ang attachment ng individual remittance is on a yearly basis..this is why may mga times na nagrerefund yung ibang employer.

hello mga sis, ask ko lang  yung philhealth ng dad ko, voluntary member siya before then naputol yung monthly contributions niya. And balak ko bayaran ulit, re-activated ba kagad yun once nabayaran na? and pwede rin ba gamitin right away ang benefits (just in case kelanganin)?

I would like to know if I can be the one to apply for my boyfriend's PhilHealth identification number? How about his signature required in the form, can i affix mine on his behalf?

He's a seafarer and is already aboard their ship. We were only informed by their office that he has to have his philhealth number, the day before he was about to leave. They advised me to submit his birth certificate for the processing of his philhealth number but when I went there, i was told to be the one to get the philhealth number. They gave me the Er2 form. So, is it possible if I'd be the one to get it for him?

seaman din ang asawa ko. meron sya philhealth e kaso wala ko alam dun kasi mag bf pa lang kami nung nagsimula sya magbarko. ngayon gusto ng asawa ko na kami ni baby ang ilagay na dependent..

di nya alam philhealth number nya. ano ba gagawin dun? dko alam pano sisimulan.. kulit ng asawa ko e. e sayang din naman..

Hi Nuna, alam ko pwede mag-update ng dependents. Fill-out ka lang ng form then attach mo birth certificates ninyong 2 ng baby mo. Ang hindi ko lang alam, kung pwede ikaw ang mag-update. Ask mo sa office ng hubby mo kung ano philhealth number niya.

Hi Nuna,
I think as legal wife you can file your hubby's philhealth and update his list of dependents on his behalf. It is easier din if you do that sa Philhealth office sa poea building mismo.
They can check na rin what is your husband's number doon kasi online na sila.

yes po naayos ko na mabilis lang pala chaka wala naman tinanong sakin, process na agad nila. Kasi meron naman ako ng Er2 form galing sa company ng bf ko.

ask ko lang, makikita ba sa philhealth yung mga previous employer mo?

ganito kasi. di ko dineclare yung last employer ko nung nagapply ako dito sa current company ko. ngayon hinihingi ng HR yung previous employer ko. di ko nga lam kung bakit eh nasa kanila naman yung resume ko..

tingin nyo ba dapat ko sabihin yung tunay na last employer ko o kung ano na lang yung nasa  resume ko? kasi kung sasabihin ko yung last employer ko, ok lang sa philhealth yun kaso sa HR namin baka ipull out yung record ko tas wala dun yung sinabi ko.. or wag ko na lang sabihin yung latest, kung ano na lang yung nasa resume ko, para di na ko tanungin sa HR kaso di ko alam baka makita sa Philhealth na may iba pa ako emplyoer. 3 mos lang ako dun sa last tapos 3 years na ko dito sa current ko.

^ako din, nung nag-apply ako sa current employer ko...hindi ko dineclare yung company na pinasukan ko before kasi 3 months lang din ako dun chaka AWOL ako. hindi din nakalagay yun sa resume ko. pero hindi naman hinanap ng HR yun sa akin. Ibigay mo na lang sa HR yung naka-declare sa resume mo. I think, wala din naman sila makukuha dun sa totoong previous employer mo kasi 3 mos. ka lang din dun di ba?

hi mga sis! ask ko lang pano pag nawala [textspeak!] MDR. pano ba ko magrerequest ng bagong MDR? eto pa din ba [textspeak!] form na gagamitin ko (PMRF) kaso nakalagay kasi dito [textspeak!] purpose ay for enrollment (new member) and for updating lang.

sis seduceme, punta ka lang ng philhealth office at mag fill up ng form just inform the help desk na i-continue ang sa father mo. after 3 months pwede mo na gamitin for emergency purposes. for maternity or operation naman maghihintay kapa ng 9 months.

^i think so. sa las pinas branch kasi ako nag-update when i transferred companies. eh, as far as i know, yung previous employer ko sa main branch sila nagre-report.
