Unmarried: Newborn Birth Certificate

PS: first time mom here kaya I have lots of questions hehe

I will be giving birth sa St Lukes QC and di pa kami married ng daddy ni baby. Pero we want to use his surname na since nasa plan na din naman [textspeak!] marriage not long after. I havent read posts explaining [textspeak!] whole process sa ganitong case. But here's what I have gathered (sana may magcorrect/mag-add ng info)

1. sa nabasa ko, magbibigay ng form [textspeak!] ospital (st lukes) para dun sa info like name ni baby
question: ilalagay ko nalang ba agad [textspeak!] surname ng daddy nya dun sa form? or under ng surname ko muna then papalitan nalang?

2. I have read kailangan ng Affidavit of Paternity tska Affidavit to Use the Surname of Father, saan kukuha ng ganito?  ano [textspeak!] mga papers na kailangan namin dalhin aside from the two?

You need to fill up the form. You can use the surname of the dad as long as he sign the second page of the birth certificate na bibigay sayo. They will also ask the latest cedula of your partner.

Yan yung sa second page. Just ask your partner to sign on it.  Plus his latest cedula.

Pls note na illegitimate pa rin status ng bata ha since di kayo married but your baby can use the father?s last name as long as he signed na acknowledgement page na your baby can use his last name.  Hope this helps

^^may forms yung hospital and sila na mag asikaso ng registration ng birth cert. ang importante the dad is present and acknowledges the child. He needs to sign the forms. Bago naman kayo ma discharge ipapagawa na nila sa inyo yun and kung may need dalhin like id or something you will be informed.

Kung pwede and abot pa maybe you can get married before giving birth para dun sa remarks legitimitate nakalagay and no worries na sa pag papalit nun. Medyo hassle din kasi palitan after. Since sabi mo nga magpapakasal din naman kayo.
