Well sis, the only way to find out is to ask.
For me, what I do is I open up the topic by telling what I feel. Like in this situation, i'll probably start it with telling him na you want to trust him fully again and that you need to clarify some things prior to moving forward.

But when you do this, you have to take his answer and give him the benefit of the doubt.
that is if you feel that he deserves it.
So take your time and really ask yourself if you can trust him again and start a new chapter.

Confront him directly. Give him the old fashion "(Name), we need to talk" approach and he'll know you mean business. Make sure to keep eye contact so you'll know if he will be lying or be truthful to you. Don't be scared. Guys like it when girls are direct to the point.
