^Sa Dswd office ng city hall ng caloocan. Sa mga city hall may dswd office, kuha ka ng form kung saan city ka sakop.

Requirements:
1. brgy clearance (nagrequest muna ako ng endorsement letter sa purok leader and cedula sabihin mo gagamitin pagkuha ng solo parent id). Within the day nakuha ko ito.
2. 2 2x2 pic and 2 1x1 pic.
3. affidavit of separation or abandonment (pero di na ako hiningan niyan siguro kasi dahil hindi kami married ng ex ko)
4. Proof of income (nagsubmit lang ako ng 1month payslip--pwede ITR/COE)
5. Birth certificate ng bata

Thanks sis!  Would you know kung pwede din silang mag issue ng Travel Clearance (this is for my brother, hindi kasi namin kasama ang mother ng pamangkin ko  pag-alis e and hindi sila kasal).

^Not sure kung nagi-issue din sila ng travel clearance, di ko naitanong nito. Yung kakilala ko kumuha siya 3yrs ago dun daw sila sa dswd field office sa Sampaloc pinapunta.

Ito yung phone # na binigay niya sa akin ng Sampaloc field office (02)313-1435 loc 210. Di ko sure if ok pa yan number kasi di ko pa na-try tawagan.

^Inquire na din kayo sa Dswd sa city hall bakasakali na nagi-issue na sila. If sa field office pupunta brother mo make sure lang daw na agahan punta and complete na ang requirements para makuha niya ang clearance within the day.

I called DSWD Sampaloc Field Office yesterday.  Ang sabi ng nakausap ko, Legarda, Malate and Alabang lang ang field offices ng DSWD sa Metro Manila and sila lang ang nag rerelease ng Travel Clearance for minors (meaning, wala sa mga city halls).  I asked kung updated ang requirements list na nasa website nila and unfortunately, hindi so tinanong ko kung anong mga requirements... and eto po sila:

Requirements:
-   Application Form
-   Minor?s Birth Certificate (NSO and photocopy)
-   Mother?s CENOMAR (NSO and photocopy)
-   Mother?s Birth Cert (NSO and photocopy)
-   Affidavit of Consent & Support of the Mother with the following details:
o   Name of Minor
o   Destination
o   Purpose of Travel
o   Name/Relationship of Companion to Minor
-   Valid ID of Mother (Orig and Photocopy)
-   Passpor-Size ID Picture of Minor ? 2x
-   Original and Photocopy of Passport of the Travelling Companion (2 copies)
-   Photocopy of Minor?s School ID

^Thank you sa pag-post nito sis. Layo pala ng pupuntahan namin kung ikukuha namin pamangkin ko ng travel clearance next year.

Sana i-update nila ang requirements nila sa website, napakadependent pa naman natin sa internet ngayon. Kawawa yung pupunta dun na nagdepend lang sa requirements na posted sa website tapos kulang, babalik pa siya.

Hi! I know my situation is different.

I take care of my nephew (his last name is my middle name). And I want to bring him to places I have seen.

I was thinking of local travel lang muna. Manila to Boracay siguro... Has anyone done this and how did you deal? What did you prepare to make sure that the kid gets to ride the plane and go home?

(I have a friend who has the same case but she filed legal guardianship eh. I am not a legal guardian. I am just an aunt who took my nephew in so he can study and eat regularly).

Last week, nag-apply ako ng Travel Clearance for my nephew... kung hindi lang sana absent yung OIC, nakuha ko din sana on the same day yung TC .

Pwede ko ng kunin today but I'll get it na lang next week.

Pina submit pala ako ulit ng Affidavit of Consent from the mother kasi hindi match yung signatures sa ID and sa consent.  Though hindi naman namin dinaya pero kasi may konting difference ang dalawang signatures, ayun.. pinaulit ko sa nanay.... panibagong notaryo na naman.... pwede ko naman daw isubmit sa pagkuha na ng TC.

^Sis need din namin kumuha ng travel clearance for my niece. May format ba ang consent or naida-download ba ito? How long is the validity? July pa ang travel date namin.

^^Kung within the country lang naman no need to worry sa pagbring ng nephew mo for travelling. We?ve done that niece ko, no questions asked naman ang mga airline personnels. Pero kung ilalabas mo siya ng bansa then that?s a different story.

Wala namang format pero ginaya ko yung ginawa ng blogger na to and when I called DSWD to ask for the requirements, binigay naman nila kung ano-ano ang mga details na dapat nasa affidavit.  I think I posted here in my previous post

For first-timer, 1 year lang ang binibigay.

By the way, hiningan din pala ako ng kopya ng airline ticket namin buti na lang may dala ako kasi hindi naman sinabi nung tumawag ako sa kanila before.
