what's the full story on the little girl pala? did she get bitten accidentally or nagwala talaga ang dog nya? when did the manifestations start to show? and what happened to the dog?
Nakagat na rin yung nephew and kasama namin sa bahay  ng dog ko, very light scratches lang. one of my dogs is super hyper kasi, talagang na eexcite when someone goes up and down the stairs and minsan he nips the ankles of visitors.  Hindi ko pa talaga na fully correct yung behavior nya na to 

but they usually stay inside the house kasi, and seldom interact with there dogs so hindi kami masyadong worried. only pag pinapasyal ko sila sa park do they meet and play with other dogs.

^sis you can have yourself vaccinated pre exposure tawag namin dun, so once you get bitten booster nalang.. you won't need yung immuneglobulin na vaccine which is very expensive and super painful. if ever ypu get bitten.

very rare ang vaccine failures sa dog regarding the anti rabies shots, pero there are still incidences.

the girl kasi, syempre scared did not tell her parents about it. yung bite ay nasa 4th digit ng right foot. we couldn't get the full history since nahirapan na sya to breathe. hydrophobic and aerophobic narin. the parents only found out about the bite when she had symptoms na... too late kaya kawawa.

last night naman, there's this rabid patient nasa 30's, yung wife nya inaaway kami bat daw naconfine pan wala naman pala kami gagawin but to just wait for him to expire. kainis lang. kasi they brought this patient to a manghihigop, this manghihigop claimed na nahigop na daw nya ang rabies. tsktsk.

^where would you recommend to get the pre-exposure vaccine?  And how much would it cost?

It's really is quite scary if you think about it.  My other nephew was scratched/nipped  by a stray cat last year when he was around two y/o.  buti kasama yung yaya so nakita agad, pina inject nila kaagad ng anti-rabies. ilang shots din yun, but thankfully ok lang yung nephew ko.

I can understand how frustrating it is on your part and the rest of the medical staff pag maka encounter ng cases like that of the 30 yr. old patient. Bakit pa kasi maniniwala sa mga witch doctor 
Usually anong ginagawa nyo sis to ease the suffering of the patient?  Do you give a sedative or something?

chubbelita, lets say a person got bitten (yung tiny scratch lang or size ng pinhead) and bled a little, tapos di nagpa shots pero wala naman nangyari sa kanya or sa dog.

 Is it safe to say na ok lang sya?

Ang ano bang timeframe ang masasabing he's out of danger/risk na of getting rabies Kasi he didn't show any symptoms.

And for those that got bitten, ano naman timeframe to get the necessary shots?

Ps: yung previous comment ko, I meant to say na completely No bleeding, as the original poster said. Teeth marks and pasa/hematoma lang as I understood it ang meron yung victim.  But still the kid was given shots.

As long as may open wound regardless sa size ng bite dapat talaga magpainject since rabies is deadly once activated na sa victim.

Also, hindi lang po sa rabid dog nakakakuha ng rabies ang pets natin even sa bats pag nakagat sila.

may nababasa pa nga ako even scratch ng teeth may ibang doctor inaadvise na magpainject padin ang patient pati pag scratch ng nails.not sure lang if it's really proven or just to make sure lang na magiging 100% safe ang victim since there's no cure pa sa rabies.

And yeah once nakakagat ang pets natin it is our trsponsibility to pay for the medical expenses ng victim kahit sa loob pa ng property natin. Pero there was a discussion before sa fb about that, ang sabi ng isang lawyer if proven na trespassing sa property yung victim kaya nakagat ng pets natin then pwede ilaban para di sagutin ng pet owner ang expenses.

animal bite has 3 categories

cat1: lick on non open wound
no management needed

cat2: minor scratch, NO bleeding noted

A. PCEC(Rabipur) or PVRV(Verorab)

cheaper - Intradermal route
days 0, 3, 7, 28/30

expensive - Intramuscular (preferred by private clinics or for those who will go to abroad in the course of treatment)
days 0, 3, 7, 14, 28/30
*same efficacy for intradermal and intramuscular vax

B. Tetanus toxoid

C.
cheaper - ATS Anti Tetanus Serum (with skin test)

expensive - TIG Tetanus Immunoglobulin (w/o skin test / for those who has positive skin test on ATS)

cat3: scratches/bites WITH bleeding
same management with category 2 plus

D.
cheaper - ERIG Equine Rabies Immunoglobulin (with skin test)

expensive - HRIG Human Rabies Ig (w/o skin test / for those who has positive skin test on ERIG)

*both are expensive cheaper lang ang ERIG dahil from horse. expensive din dahil once lang iinject unlike pcec/pvrv na 4-5x iinject. this can be delayed up to 7 days ONLY. if you choose Intradermal route, pwede to ipabigay until day 7 only if wala pang funds ng days 0 and 3.

para saan ang 14 days observation?
it is only for the animal, not for the human's condition. if on the 15th day the animal is still alive, it is considered non rabid. you can have peace of mind and the last day of your injection(day 28/30) is now optional to be given. but my advise, ipainject na rin. but if within that 2 weeks period, the animal died, you have options. 1. send the head of your dog at ritm to confirm if it died of rabies, if negative, same as above. if positive or you cant go to ritm, you are required to finish the treatment until day 28/30

what if you have open wounds needing stitches?
after ERIG/HRIG administration, 2 hours waiting period is needed before suturing of open wounds is to be done

^nakapagwork ako sa animal bite sis. hindi ko na lang binanggit ang price kasi medyo limot ko na. banggitin ko na rin para may idea ang iba, hindi ko sure kung nagincrease na ba. p460 for pcec/pvrv. tetanus toxoid is free. ats around p130+ per ampule. tig 700+. erig 1400/vial. hrig 2000+ per vial.

where do you work sis? ive read you have seen rabid patients. kawawa di ba kaso wala na talaga magagawa eh. nakapaginject ako before sa rabid patient ng day 0 niya. on his follow up, ibang nurse, sisinghap singhap na. nagsinungaling sila sa kwento nila. sabi ng relative 2weeks ago nakagat. ang totoo, months na. at kaya pala very eager ipasaliva pcr kahit uber mahal kasi naghihinala na silang may rabies. nagbooster na lang kaming nakahandle sa kanya..

but regarding the pre-exposure vaccine how much would it cost sis? and isang shot lang cya?  wala cyang side effects?
isang option lang ba sa pre-esposure or same lang din sa post exposure na may two options?  and in this case granting na makagat nga after the pre-expoure shot(s)  you have to use the same type/brand of vaccines din?
sorry talaga for all the questions sis ha. just want to make sure lang, mahilig kasi ako mamulot/mang rescue ng stray cats

thanks sis for reminding me about pre exposure vaccination. i advise moms to have their children vaccinated of pre exp vax. sana isama na yan sa EPI. heart breaking makakita ng batang namatay dahil sa rabies. lalo na yung hindi nagsabi sa parents..

benefits of pre exposure vax?

once a person is bitten but completed the course of pre exposure vaccine regardless if it's category 2 or 3, the person doesnt need all those rabies vax but just a booster vax plus tetanus injections. schedule is day 0 and 3, 1 intradermal injection only. just make sure to bring your booklet of pre exp vax to serve as a proof.

iadd ko about HRIG, this is advisable to be given despite being tested negative on ERIG if the person is bitten on head area. may times kasi late allergic reaction. sobrang onti kasi ng gamit pang skin test unlike sa iniiject sa patient na ilang ml. but because of its price yung iba Erig lang ang afford but they sign a waiver that the consequences are explained. and as much as possible if the bite is on head, vaccine is to be given within 24hrs. if the person dont have money for erig/hrig, at least maadminister ang pcec/pvrv. kapag nakagat from shoulders pababa, do not panic. you have 7 days for vaccine to be given. wag ng pumunta ng hospital dis oras ng gabi. ipagpabukas na. you wont die of rabies.

and also erig/hrig is injected near the bite site. palolobohin talaga ng kaunti. ramdam naman ng nurse if hindi na kaya ng muscle. ang tira sa legs na..

mura lang pala pag pre-exposure vac. will ask my cousin about this since she works at a lab at a nearby hospital.  hope di naman super mahal pag private.

one more Q sis, after one has gone through a full course of POST exposure vaccinations ba, you only need booster shots later?  like my nephew's case, he was vaccinated last year after a cat scratched him. so if another incident were to take place, booster shot na lang, or full course pa rin?

hay, buti nga hindi kami na expose sa rabies.  since i was a kid kasi mahilig na ako mangkupkop ng kung ano anong animals- cats, dogs, birds, etc. and many times na rin ako na scratch.  In high school, my best friend was also animal crazy, she even rescued a baby bat once (scary pala now that i think about it) and her bro worked at a petshop, so we had no qualms in handling all sorts of animals. Thank God we didn't get bitten

tama ka sis. booster shot na lang hindi na full course. kaya dapat itago ang booklet. basta sis kada may bite, booster shots kahit pa 2 weeks lang nakagat na naman. lifetime ang rabies vaccine. hindi na kailangan pang bumalik sa clinic after 3 or 5yrs for booster daw. gawain kasi yun ng ibang private clinic. hindi naman sa naninira ako. kung walang new bite, no need for booster. do not worry sis kung ang ginamit sa full course ay example Rabipur then sa booster Verorab. okay lang yun.

yung rabies vaccine hindi lang siya sa dog, cat or bat. pati kapag nakagat ng livestock needed din. hehe. kapag nakagat ng human and rodents ang hindi. nakakatawa nga kasi may naencounter kami nagpaparabies vaccine dahil kinagat daw ng classmate ang anak niya. lol..

hi sis pinkswirls. i work at ritm:) this week alone we had 3 rabid patients. kawawa the other day, 4 year old girl from cavite. yung parents they knew about the bite, kaso went to the albularyo instead. she died kagad after a few hours.
