regarding SSS maternity pay (thread 5)

ART. 133. Maternity leave benefits. - (a) Every employer shall grant to any pregnant woman employee who has rendered an aggregate service of at least six (6) months for the last twelve (12) months, maternity leave of at least two (2) weeks prior to the expected date of delivery and another four (4) weeks after normal delivery or abortion with full pay based on her regular or average weekly wages. The employer may require from any woman employee applying for maternity leave the production of a medical certificate stating that delivery will probably take place within two weeks

tama po ang pag ka intindi ko dito? kung 2 months kang leave, yung first month may sweldo ka na buo tapos bukod pa ba yung makukuha sa SSS? salamat.

^ sis, if Im not mistaken (and based na rin sa twice kong pag-maternity leave) and suswelduhin mo lang is yung mga araw na nag-work ka. Yung SSS maternity benefit, that's to cover your 60 days of leave.

Ano bang mga need na papers after manganak?

Kasi before manganak, need magsubmit ng MAT1 sa SSS, if employed, HR na ang bahala sa process.

Add ko na din na kumuha na ako ng Philhealth MDR, certificate of contribution,at CF1.

After manganak of Normal delivery, ano yung need makuha at submit sa SSS/employer?

If CS, ano din yung papers na need maliha from OB and hosp?

^^ If CS, Operating Room Record, should be Certified True Copy from the hospital plus original or Certified True Copy from the registry office ng birth certificate ni baby. And SSS ID, if wala pa then 2 valid IDs with the acknowledgment stub na nagpa-ID ka na.

Yan yung mga hiningi sakin before ng office.

Settled na Dec 14. 10 days lang approved na ang SSS Check ko. Pero hindi ko alam if I can claim it sa post office.

Nagtataka ako sis hindi maximum ang benefit ng SSS sa akin pero max ang contributions ko and sinabi din ng officer sa akin sa SSS na maximum yung makukuha ko. Nakakapagtaka.

Nareceive ko na yung full amount ng benefit..assuming normal delivery.
 
I wonder if need pa ng Mat2 if normal del ako aside from ob history at certi true copy ng Birth certificate.

^ Yes, NSD or CS, required to submit ang Mat 2.

2. Si employer ba mag bibigay ng MAT2? o mag DL ako then ipapasa nalang sa kanila?
3.yung Operation record kailangan ba na original?

1.Saan kinukuha yung Complete OB History? may download pa ba na form na kailangan ipa-sign kay ob? o mag request lang ako sa kanya?  ----Usually kasama na yung form na yan sa MAT2 ask ka lang ng copy sa SSS branch.  Last time I checked wala nito sa SSS website kaya hindi ako nag submit nyan.

2. Si employer ba mag bibigay ng MAT2? o mag DL ako then ipapasa nalang sa kanila? --- not sure sa employed if employer ba kase nag resign na ako.  Pero you can download this form or get one from SSS branch.

3.yung Operation record kailangan ba na original?  --- I assume CS ka rin?  I submitted yung original copy

^^ - You can download the form then print it and have it signed by your OB.


- MAT2 can be downloaded din from the  net, print it then fill it out.


- Operating Room Record, yung pinaka-original copy kasi talaga niyan na mismong sinulatan ng OB eh for the record keeping ng hospital yun, what you will be getting from the hospital if you ask for the Operating Room Record is photocopy na lang BUT should be stamped Certified True Copy with signature when you get it from them.

thanks sis! i've heard kasi dito sa isang buntis sa office namin na 3 months daw siya mawawala. eh halos kasabayan ko din siya.. i tried to search tho nakikita ko nga is nasa 2nd level palang ata.
