^ sis, I think that quote applies regardless kung employed or voluntary member. Ibig sabihin lang is you cannot claim 2 benefits (maternity AND sickness) for the same period or same incident.

You may continue paying as a voluntary member naman, icheck mo lang dun sa payment form na voluntary member ka. As to the amount, meron schedule ang sss. You may continue to pay the same tier as when you were employed, or you may choose to increase or decrease, pero by 1 or 2 levels lang yata pwede.

Napasa ko na ang Mat 1 ko kaninang umaga. Sinubmit ko lang yung Mat 1 form, inencode lang yung mga nasulat ko tapos binalik na sa akin yung form. Nakalagay sa form ko is encoded. Resigned na ako from work since August last year. Iaattach ko kasama ng Mat 1 is yung Mat 2 after manganak daw, kasama narin dun ang photocopy ng ultrasound kasama narin ang cert of no advance payment, separation at photocopy of L501.

Hi mga sis, pahelp naman please. Di pa ako tapos sa pagback read e. Member ako ng SSS for 9 years but last year nag stop ako magwork to be a housewife. I did not continue paying for my contribution kasi wala naman ako income. If I will avail of the maternity benefit this September 2015, dapat nakapaghulog ako ng at least 3 months from period April 2014- March 2015. When I checked with my ex-employer may payment naman from April to August 2014 (pero hindi pa posted sa SSS website ang payment from June to Aug 2014). Nakita ko din sa website na yung previous maternity benefit claim ko (for my miscarriage) was denied pero inadvance ng employer ko ang benefit na to dati at wala naman sila sinabi sakin na denied. 
reason: employer not enrolled in the smb payment thru the bank program. please submit properly filled out enrollment form to your designated *wrong key-encoding

questions:

1) i need to go to an sss branch to submit my mat 1 form. do i need to inform din na separated nako from my employer? pag separated wala naman ako kailangan bayaran diba, unlike pag voluntary na may monthly contribution na? any sss branch ba or dapat sa servicing branch? di naman maaaffect ng pagstop ko magpay ng contribution yung benefit na due sakin kasi na meet ko naman un required 3 payments diba? kailangan din magsubmit ng certification from employer na no advance payment and separated na, based sa post ni sis marieisabel. ano yun L501?

2) sa mat 1 submission, ultrasound lang ang kailangan diba, need pa ba ng SSS ID? kasi hinihintay ko pa dumating yun UMID ko and yung claim stub ko nawawala. =(

3) may effect din kaya sa pagclaim ko this sept yung denied claim dati? that was 2013 pa.

Mat 2 sinasubmit pagkatapos na manganak and it should include the completed Mat 2 form, the OB history sheet na signed ng OB na nagpaanak and the birth certificate ng baby....

Mga sis, may nakapagclaim na ba sa inyo ng sickness benefit nung nabedrest kayo while pregnant? Sinabihan ako ng SSS call center na pwede pero after checking sa website kailangan within 5 days makapagsubmit if home confinement lang.

regarding duon sa MAT 2, yung L501 magrerequest ka sa company mo. sila na bahala sa L501, may mga signatures na yun ng mga kataas taasan pati narin yung no advance payment. husband ko na ang kumuha since hindi na ako makabyahe ng malayo. hiningan nalang ako ng authorization saka id para maclaim niya.

^ no sis. actually binago namin nalang yung start date ng bed rest ko. kasi if beyond 5 days, magpapaliwanag ka pa sa kanila bakit late mo na submit. pwedeng ireject nila yung claim din, dahil sa reason.

2 weeks nilagay ni OB dun sa akin pero if indefinite ang bed rest, magffile ka ulit ng bago na extended ang bed rest mo (ang alam ko). kasi kailangan may from and to na date para macompute nila yung days na babayaran ka ng SSS. hindi yata pwedeng wala.

di ko na ginawa yung extension, hassle e. bale 2 weeks worth nakuha ko amount lang. naka bed rest ako from 15w until mag 37w. i don't know ang maximum amount na babayaran ng SSS sa bed rest.

Hi sis aiah, thanks for the reply. Ganun na nga din plan namin gawin kasi ilang weeks na bedrest ko. Ang weird ng SSS kung bakit within 5 days nasubmit na e nakabedrest ka nga kaya di ka makakabangon at di mo maaasikaso. Pano yung mga nagiisa kelangan pa maghanap ng mag aasikaso for them. Sometimes, mapapaisip ka kung bakit ganon kastrict parang ayaw nila magpaclaim ng benefit. Buti yung sa call center mabait at sinabihan ako.

mga sis tanong ko lang. kasi ofw ako so contractual ang trabaho ko. naka indefinite leave ako so pag ganun walang hulog kasi di naman ako nagrenew ng contract pero i'm still with the company. naghulog ako ng 3 mos para kahit paano may makuha akong benefit sa sss (voluntary contribution) pag ganun ba pano ifill up yung  MAT2 form since employed naman ako pero voluntary contribution yung huling hulog ko? tia!

sis I asked my mom to submit it for me. Gumawa lang ako ng authorization letter and had her bring my sss id just in case hanapan siya.

may instances ba na na de deny yung claim ? Coz until now I haven't received it yet. I gave birth last Sept 2014 and it was filed since then. My office is up to date with the payment and  I have submitted the necessary docs as well .

@freelancer sis check mo yung back portion ng sss form mo within 90 days lang aftet mo maconfirm na buntis ka.. Pero ako 4 months na mahigit tyan ko depende aa officer na tatanggap nun meron strict meron hindi..

Mga sis di naman necessary na nso birthcertificate di ba?
