• Pregnancy test before and after miscarriage/abortion.
• Medical Certificate/Obstetrical history indicating the number of miscarriages duly certified by the attending physician
with his license number, printed name and signature; or
• D & C Report duly certified by the authorized hospital representative where the member was confined.

pero nagdedepend pa din yan sa doctor ng sss kung may hihingiin pa siyang additional requirements. kasi ako sinabihan ako na magpaultrasound ulit at maghingi pa ulit ng medical certificate sa ob ko.. kahit na 2 days ago pa lang ang ultrasound at MC ko nung araw na nagpunta ako ng sss eh hindi na ko nagreklamo at sinunod ko na alng sya.

nag submit ako ng mat1 sa HR namin, last month ata yun, tapos chineck ko online, no maternity notification pa din nakalagay. ano ba gagawin ko dapat?

Sis aquamomma okey lang po ata yan kasi sa akin din nangyari sa akin eh..try to call nalang their hotline para maka sure ka .. 917 777 ang 24/7 nilang hotline..mas maganda tumawag early morning or evening na para makapasok agad tawag mo..lagi kasing busy line nila kapag regular hours eh.. HTH

^^ sis, i think important that it says you have maternity notification. That's another drop down/tab if i remember right. Same tyo ng nakikita sa page. Since we haven't made a claim for maternity benefit, talaga atang you wont see it there yet

unfortunately, In my case yung company namen hindi nakapagbayad since August 2013 and they said na babayaran nila but i don't think that's possible. Anyway, I was confined last May 31 magffile sana ako sa HR then yun yung sagot. May pwede ba akong gawin mga sissies?

Thanks sisses, maifollow up nga yung Maternity ko sa HR namin. Problem ko nga nung nag file ako e di pa ako kasal - e NSO Marriage certificate na daw ang need ko para maupdate yung SSS ko.. Di naman kaya ako magkaproblema dun?

hun_nie08 Its no a requirement naman that you are married before you can avail the maternity leave. Mandatory pa rin sya.

The full payment of maternity benefits shall be advanced by the employer within 30 days from the filing of the maternity leave application.

Sis gharz412, were you able to file MAT1 (maternity notif) with your company's HR before the miscarriage? If yes, I think they are obligated to give you the maternity benefit since it's their responsibility to file it with SSS. Negligence on their part if they weren't able to submit your documents on time.

hi sis its depend on a company policy others would give you an advance na na 30k on your 7th month kasi what if may instances na mapaanak ka na ng wala sa oras..other company naman usually 8th months..better ask your HR for sure... then sis file ka na ng change of status na from single to married and the change of surname..para just incase ipapersonal na ipalakad sayo ng HR nyo pwede ka pa sa special lane...ganun kasi nangyari sa akin ako mismo naglakad ng change of status sa SSS;Philhealth;Pag-ibig and BIR... special lane ka palagi kasi preggy.

(out of topic) add ko lang sis sa philhealth mabilis lang pmrf+marriage contract nso photocopy lang; pag-ibig pwede online tapos ipapasa mu lang yung photocopy ng MC plus yung print out sa pag-ibig na change of status sa sss fill-up ka ng form plus kelangan mung ipakita original NSO MC plus photocopy...

Oh...ang rule is once you have your first ultrasound, you have to notify HR or SSS about it. If HR didn't report your SSS contribution, liable sila kasi you get deducted every month. Pwede mo sila habulin dun if you were able to notify them about your pregnancy. May alam akong case na nag-miscarry sya before nagnotify sa HR or SSS. Pinagalitan sya ng HR na negligence on her part not to have reported her pregnancy ASAP pero dahil naawa sa kanya, tinanggap pa din nila yun MAT1 form dated before the miscarriage. Sinabi na lang ng HR sa SSS na sila (HR) ang negligent sa filing. Baka pwede mo kausapin HR nyo to help with you with that.

Kelangan ba magpachange status muna bago makapagfile ng MAT1? I heard rin kasi na kelangan rin may SSS ID bago makuha ang benefit eh wala pa akong ID kasi gusto ko sana yung married name na gagamitin ko para isang lakaran na lang.

^ sis kung wala ka pang ID pwede ka mag-apply sa sss. if ever na magpafile ka na ng mat 2 pwede mo naman ipresent yung claim stub ng id mo kung hindi pa nadedeliver sayo.

Oh. ako kasi kakakuha ko lang ng Marriage License namin e. Magkakaroon kaya ng conflict kung ifile ko na as married ang status ko pero ang ID ko is Single pa rin? :-(

^wala pa din akong ID sa married name ko sis.
Pero yung files ko, married na.
Nakapagchange status ako sa SSS before ako magfile ng notification sa kanila.
Ganun din sa ibang agencies, pag nagpepresent naman ng ID wala naman ako naging problema.
pati sa HR namin yung valid ID for notification ko single pa rin ako nun. Pero tinanggap naman.
