regarding SSS maternity pay (thread 5)

Hi! I already posted this under Change of Civil Status topic. Parang dito pala suitable ipost ang questions ko. 

Hi mga sis! Please help po, Im on my 9 weeks of pregnancy. Sa August pa po nakasched ang kasal namin, it says na after 10 days makukuha na sa munisipyo ang CTC ng Marriage Contract. My problem is, I haven't filed my Mat-1 yet, next week pa lang ako maghuhulog ng para sa Apr-June na contribution ko. Ang last na hulog ko po is when I got pregnant with my 1st baby, August 2010 po ata last contribution ko. I am a stay at home mom since I got pregnant last 2010. I do not have any valid ID except my SSS Card, of course, with my maiden name. I also cannot file my Mat-1 coz I have fears na baka magkacomplication kapag nagsubmit na ko ng Mat-2. I need to use my married name in the hospital para macover ako ng hmo ni hubby at philhealth. So ang mga isusubmit kong records with Mat-2 surname na ni hubby gamit ko.

 Alin po ba ang dapat kong unahin? Mat-1 or e-4 for change status?

I need to process my new SSS ID with married name on it para sa Mat-2 ko right? E antagal po bago dumating ang card. Please help po.

sis zachamac2011 change status ka muna, para pag-file mo ng mat-1 you can use your married name na, mabilis lang naman yung pag-update ng sss.

Hi mga sisses, Nagstart lang ako maghulog as a Voluntary Member nung January 2013. I pay 520 monthly. Then come March, I got pregnant. Last week I submitted my Mat1 na. Meron kaya ako makukuha kahit kakastart ko lang maghulog and maliit pa nahulog ko? So Jan- June 2013 tig 520 binabayaran ko.. Makakatulong ba kung bayaran ko na til December kasi baka 1st week of November ang due date ko. Will it matter kung taasan ko ang hulog ko to 676 ? tataas din ba makukuha ko? Nakapanganak na ko! Yey! Fafile na ko ng Mat-2 diba? Ano na gagawin ko basta punta lang sa SSS, dala 2 valid IDs and discharge summary? Pano kaya to nawala ko kasi yung acknowledgement stub na e6

^congrats sis tellycopter! Di ba pag walang SSS ID ok na din naman yung 2 valid ids? Compulsary ba talaga na may SSS ID sa pagclaim ng benefit?



Mga sis! Question lang. Gaano katagal ba makikita sa website na nagfile ka na ng maternity notification or Mat1? Kniclick ko kasi yung Maternity Notification link sabi dun (Member has no Maternity Notification) Eh nagpass naman ako ng Mat1 ko nung July 18.

Hi sis! May makukuha ka naman. Bale yung computation e ganito:
Katumbas ng 520 contri mo in terms of monthly salary credit is 5,000.
So 6 x 5,000 = 30,000 (total MSC)
30,000/180 days = 166.67 (Daily Maternity Allowance)
166.67 x 60 days = 10,000 (benefit if normal)
166.67 x 78 days = 13,000 (benefit if cesarean)

Hi mga sis. First time mom here. Nagsubmit lang ako ng  mat1 thru online and lumabas naman notification after a few days.

Gusto ko lang clarify kasi wala tlaga ako idea, ang next step na ba is wait lang ako till makapanganak and saka ako magfile ng mat2? And after that, dun palang nila ipapadala ang check?

sis tellycopter - congrats!  kelangan mo din ng CTC ng birth certificate ni baby. sayang pila pag wala ka nun. pg nsd birth cert and e6 stub lang ata hahanapin saka ids. pag cs same reqs but need pa ng surgical memorandum or operating room records authorized by the hospital.

talaga sis shenggay? naku im worried, one week na kasi since I filed wala pa rin nagrereflect sa account ko sa sss. What should I do mga sis? Tingin niyo ba dapat pumunta ulit ako sa SSS just to make sure? Ibinigay din ulit sakin yung Mat1 form ko na may tatak ng employee na received. Pero still.. Baka hindi naman pala nafile. Haha. coquettish - i don't think you need to go to sss. minsan kasi, hindi naman nila naa-update yung website. pinakaupdated lang dun e yung contributions.

saber15 - yes. after ka manganak, saka ka submitng mat2 and other requirements, then saka idedeliver yung cheque.

mga sis sorry ah pero i have to bring the mat1 sa OB diba pag nagpa check up ako before filing it?

what i did is, when i had my 1st ultrasound i got mat1 from our hr and filled it out. attached the ultrasound and went to sss for filing and brought it back to hr. but if your working,(i resigned from my prev job kasi that's why i processed it myself) hr can do it for you by submitting mat 1, ultrasound copy and letter of authorization for them to file it to sss for you.

found out i was pregnant in january. by april, nag-sabi ako sa accounting staff namin na mag-claim ako ng sss maternity benefits -- sabi nya no problem, at covered naman.

around june, nagkaron kami (sa wakas!) ng totoong hr person. i gave her my change of status (from single to married) and the docs she needed to file my mat 1.

mid-july, nag-resign si hr! i checked sss online -- na-apply na yung change of status ko (married name na ang lumalabas ngayon). pero wala akong contributions since last year, at wala ring maternity notification!
 
i confronted the accounting staff. sabi nya, wala daw ni-turn over sa kanya si hr. aba aba aba, e as early as april nagsabi na ako sa kanya ah?! kaasar talaga.

i downloaded the mat 1 form and gave her a new copy, pati copy ng ultrasound report. nakapag-remit na daw sila ng payments sa sss last week at iniintay lang ma-update. i checked online now, wala pa rin. hopefully ginawa na nila talaga -- kasi 33 weeks na ako!

My bf wants us to get married next month, bago daw sana lumabas si baby sa october kasal na kami. Magkakaproblema kaya ako or mahihirapan ako magfile ng Mat2 ko? Kasi magchchange status ako di ba? Pls help..
