Was at DFA yesterday to apply for my 5 month old baby's passport. We brought her NSO certified birth certificate and original birth certificate just in case. We went thru a travel agent and everything was processed really fast! We were done in 15-20 minutes.

For the photo taking, they placed an empty printer box on the chair and I squatted in front of the chair to support my baby's waist, my yaya also held onto her arm from the side. Another lady got my baby's attention to look at the camera. Wasn't as hard as I thought it would be.

Hi to all newbie here, I just want to ask if we can staple the old passport with visas to the new one? since its biometric, Im not sure if it is still allowed? tia

Just want to share my experience at DFA today.  Total time spent: 3 Hours.  The line moved pretty quickly, there were just too many people.  By the time I got to the encoding/ picture taking area, I had to wait for 400 people in front of me!  Also, I got delayed at the first window because I was asked why I did not immediately change my last name when I got married.  I had to have an affidavit made explaining why my old passport was still under my maiden name even though I was already married.

Hi! same situation din sa akin,kailangan pa bang ipa notarized yung affidavit sis?ang sa akin kasi nung kumuha ako ng passport single status ang nilagay ko pero married ako,now annuled na ako and recently got married and I want to use my husband's surname na.panu ba ang processing?medyo kabado ako kasi nga may discrepancy,nung kumuha kasi ako nun sabi ng manager ko dapat single ang ilagay,problem ko tuloy ito ngaun.

^^sis ask ko lang nso birth certificate lang dala mo and orig copy? Ano pa ba kailangan sa infant?! Kailangan pa ba ng marriage contract from nso?! Ako lang kasi kasama ni baby magaapply pero sa pagtravel kasama namin si hubby.. Thanks!

One more question, pwede pa bang saturday kumuha sa dfa? Kasi naalala ko dati saturday sched namin e this time may lane for infants kaya no need na magpa appointment. Ang di ko lang sure e kung may saturday pa. Or mon-fri na lang?

Yes! Dapat notarized yung affidavit.  Napagastos tuloy ako ng additional P300 para sa notarial service inside the DFA.

If you never declared that you were married before and you never used your previous husband's last name, I don't think you would have any problem renewing your passport under your current husband's last name.  Just bring your new NSO certified marriage contract with you.  Wag mo nang e-mention yung previous husband mo para hindi magulo.

Magkaka-problema ka lang kung yung old passport mo, last name ng previous husband mo ang ginamit mo pero single pa ang status mo.   But then again, hindi naman hinanap sa akin yung birth certificate ko, so I think as long as may NSO Marriage certificate ka naman sa bago mong asawa, hindi rin nila malalaman na nag-iba ang surname mo.  They will assume na maiden name mo yung nasa old passport mo.  Just to be safe, pagawa ka na rin ng Affidavit of explanation (explaining your whole situation) at yung documents about the annulled marriage.

Thank you sis nid2sell!kahit papano narelieved ako,hinde ko ginamit yung surname ng ex ko sa lahat ng documents ko,maiden name talaga eversince,kasi hinde naman kami nagsama at nag pa annuled na nga ako and then nagpakasal,yun nga nga lang hinde ko declared na married ang civil status ko,puro single and sa bago ko kasing marriage contract, single(annuled) ang nakalagay sa civil status ko,complete yung annulment papers ko,dalhin ko na lang din pag nagpunta ako.Sis, handwritten ba yung affidavit or dun mismo magpapagawa and notarized?may interview pa ba about dun?salamat sa information,hinde ako makatulog dahil dito sa kakaisip.

Typewritten yung affidavit.  Sa loob ng DFA ko na pinagawa (doon din sa STEP 1 room), para hindi na ako maghanap sa labas at matagalan pa.

Wala namang formal interview.  Tinanong lang ako bakit hindi ko ginamit agad yung surname ng husband ko.  Ang nakasulat nga sa affidavit ko, for convenience kaya hindi ko pinalitan yung status at last name ko.

In your case, I think if you got married after your old passport expired, wala nang tanong yun at hindi ka na hahanapan ng affidavit.  Sa akin kasi, yung old passport ko, nung na-renew before, married na ako pero hindi ko pa ginamit surname ng husband ko.

AFAIK, there is a courtesy lane for Senior citizens and Families with small children.  Not sure about gov't employees.  But read these blogs:

ask ko naman dito sa mga sis, ive read sa requirements ng renewal ng passport ng kids is both parents original and photocopy is required minor or kids passport,
what if in my case ofw husband ko and [textspeak!] now nasa abroad sya pwede ba na yun sa akin lang na passport i present ko?

Traveling with guardian and either parent is in the Philippines only:
•Passport of guardian, original and photocopy.
•Original Birth Certificate from NSO in SECPA
•Original and photocopy of DSWD Clearance.
•Affidavit of Support and Consent indicating the name and relationship of the guardian. (Must be notarized) •Passport or Original ID w/ signature of the parent who executed the affidavit.
•If the father is the executor, prepare Marriage Certificate in SECPA as well.

•Personal appearance of minor with either parent. •Valid ID of the parent accompanying the minor for appearance.
•If father will appear with the child, prepare the Marriage Certificate in SECPA.
•If mother will appear with the child, no need for Marriage Certificate in SECPA.

Traveling with guardian and both parents are out of the country:
•Passport of guardian, original and photocopy.
•Original Birth Certificate from NSO in SECPA
•Original and photocopy of DSWD Clearance.
•Affidavit of support and consent indicating the name of the traveling companion and the relationship to the minor (must be authenticated by the nearest Philippine Embassy or Consulate General). •If the father is the executor, prepare Marriage Certificate in SECPA as well.

• Special Power of Attorney designating the representative by name and authorizing him/her to apply for a passport on behalf of the minor in their stead (must be authenticated by the nearest Philippine Embassy or Consulate General).
• Certified True Copy of parents' passport (must be authenticated by the nearest Philippine Embassy or Consulate General)
•Original valid ID w/ signature or passport of the authorized representative in the Special Power of Attorney document.
•Personal appearance of minor with the authorized representative in the Special Power of Attorney document.

when i applied for passport, my birth certificate was not clear daw and asked for a copy/certification from the local civil registrar. it took months before i was able to do that - i was born in bicol. when i came back, the staff did not even look at my [textspeak!] from the local registrar (sigh!)

feeling ko tuloy depende sa staff kung lulusot ang mga dala mong requirements.

Question, sino ng hyphenate ng last name dito. I'm thinking of changing my passport to my hyphenated last name. Should I assume same requirements as women changing to their husband's last name?
