Hi Sis! Try mo kumuha ng Green Card sa Makati Med.
alam ko around 18-20k yata for NSD and 34.5k for CS. *without complications* Just go there and ask where HSP (Health Service Program) is, tapos pag nandoon ka na, get a number dun sa guard sa labas ng Room 1. Pag tinawag ka, you'll be interviewed by a Social worker,
1. If you have work
2. If you have a healthcard
3. Do you have a private doctor in Makati Med (dapat wala)
Basically the questions are just to check if you qualify for the Green Card. Yung requirements na hihingin sayo will be based on your answers din pero eto yung nasa list of requirements (usually hindi lahat to nirerequire, depende sa social worker na nakausap mo ano ichcheck nya sa list) :
1. 2pcs 1x1 ID
2. ITR (yours or kay hubby)
3. Certificate of Indigency (kinukuha yata to sa munisipyo)
4. Photocopy of Philhealth card (or healthcard, if any)
5. Recent 1 month payslip (if you have work)
Ako ang hiningi sakin, yung ID Pics, Payslip tapos since may Maxicare ako, ni-require na magprovide ako ng certification from our HR na exluded ang maternity benefits dun sa Maxicare namin.
Tapos pag okay na requirements mo, punta ka lang ulit for the interview M-F 8-12nn and 1-4pm ang sched nila. usually pag complete na requirement mo, bigay din sayo yung green card that day.
Also, makakatulong din siguro if hindi ka masyado cute or mukhang prepared that day. ;-D
Medyo Madami tao kasi marami kumukuha and nagpapcheck up pero konting tiis lang
For future use, pwede mo din siya magamit for any procedure other than maternity related concerns. 30% ata ang bawas for major procedures. and libre check up din for any concerns.
