F-137/Transcript of Record for evaluation purposes from Kindergarten up to the most recent level of schooling, with official English translation, if previously enrolled at a foreign school. Documents executed outside the Philippines must be authenticated by the Philippine Embassy/Consular Officials of the Philippine Foreign Service at the place of issuance or nearest to it, with official English translations if it is written in other foreign languages. b. Valid passport with visa entry4. For Grades 3 to 9: 2 copies of letter of recommendation (using CSA-MAKATI form) from class adviser and guidance counselor, sealed and signed on the flap5. 1 Recent photo (1x1)6. Hi! Anyone here from Sucat Paranaque? May school bus service ba kayo na alam na accredited ng CSA sa area? Isa lang nakita ko and located sa Concha Cruz sa BF pa (eto din binigay ng school nung tumawag ako). Location kasi namin is bandang JAKA Plaza. TIA.
