^liz_cl, ganyan din case ng hubby ko. the time na kukuha siya ng board exam, kumuha kami ng NSO certificate. we found out na wrong spelling yun middle name niya, CONCEPTION ang nakasulat instead na CONCEPCION. we filed sa local civil registry para maayos. nag submit lang kami ng supporting documents, like birth cert ng mom niya, siblings ng mom niya, and siblings ni hubby, proving na it's spelled as CONCEPCION talaga. tsaka some school documents, IDs na ginamit niya talaga yung correct spelling ng middle name.
upon filing the correction sa city hall, we paid almost P2500 (if i remembered it correctly). after 3months, we get the petition papers (whatever they call it, i forgot na kasi) to the local civil registry, then kami na ang nag follow up sa NSO manila. i think P500 naman ang binayaran namin sa NSO. then after 3days, balik nanaman kami sa NSO manila to get the corrected birth certificate. may note lang sa baba na yung middle name niya was corrected from CONCEPTION to CONCEPCION.
^sophisticated101, may similar case din ako gaya ng sayo. yung brother ko, since birth, JOMAR LOUIE talaga ang ginagamit niya na name. then nung kumuha kami ng NSO certificate for our passport, yung first name niya is JOMAR lang pala ang nakalagay. i dont know what happened next, if inayos nila or they let it be. kasi from then until now, JOMAR nalang ang ginagamit niya. siguro para less hassle, kung ano nalang yung lumabas sa NSO, yun nalang. anyway, second name lang niya yung LOUIE. mas important yung JOMAR. :-)
another case, is yung eldest daughter ko. i got pregnant at 17, inlaws & my parents agreed that they wanted us to be married civilly kahit under age ako, just to have a marriage certificate to present by the time i give birth. so, we arranged with a judge to assist our marriage (jan2000).
nung pinanganak ko siya, feb2000, nilagay namin sa birth cert niya is surname na ni hubby, so did i, we're married. we also submitted the marriage certificate that we have.
but then, months after i gave birth, they informed us na hindi considered yung civil wedding namin last jan2000. because under age pa daw ako. so when i turned 18, we got married again (for real) by oct2000. kampante na.
again, when we were arranging documents nanaman for abroad. we have to really settle the issue kasi baka maging major problem na siya. baka maquestion yung date of marriage namin, magkaiba ang nakasulat sa birth cert ng eldest and youngest ko. so, we have a laywer with us, napakagastos.

we filed this case on june2010, 2 court hearings (1st: jan2011, 2nd: feb2011) the case was finished last feb2011. no adoption needed, acknowledgement lang ni hubby.
