Change of Civil Status

Ako nag-apply ng SSS UMID last week of November 2015, dumating last week of July 2016. Almost 8 months of waiting. Nag-stop daw kasi sila mag-print before election nag-resume lang last week of May 2016. Grabeng tagal knowing na nasa Rizal area lang ako di naman kalayuan.

Does the UMID stand for everything else--- SSS, Pag ibig, etc? I have UMID na, do I need to have all the other IDs?

^yes sis good na yun. if try mo icheck yung back portion nya, covered na nya yung SSS (or GSIS), PAG-IBIG and Philhealth:)

about UMID, pwede ba magrequest for a new ID with my married name na? Maiden name pa kasi yung UMID ko.
2013 pa ako kinasal pero until now wala pa akong gov't ID with my new surname, lahat maiden name ko pa kaya hirap din ako sa mga forms or even making reservations under my name.
my passport is expiring by 2018, pwede na ko magrenew but I don't think It's ok to bring my passport always just for identification purposes, baka mawala/masira pa.

I was working in SG nung kinasal ako, immediately informed our HR to update my record, mabilis lang nakareceive na ako notification from gov't na updated na status ko although hinde na ako nagrequest for a new ID card since hinde naman required sa SG ang change name, actually hinde uso yun sa mga locals dun. But sa Pinas, wala akong pinaupdate na record  .

I'm back working in Manila, during processing ng employment application ko, Married naman yung status na dineclare ko kaso wala akong maipresent na gov't id, marriage cert na lang pinapasa sakin. My office record and ID is my married name, they asked for my SSS, Philheath, TIN but I'm not sure if they updated my status or new name sa mga agencies na yan when they hired me.
isa-isahin ko kaya pagverify?

I'm pregnant and worried na magkagulo gulo pag process ng SSS/Philhealth even hosp bills dahil sa name ko, my hmo pala is my married name so as my hospital record.

Regarding UMID, yes, pwede naman magrequest ng new ID pero bayad ng 300 for the new ID and you need to return the old one sa kanila. Better start updating now para pag due date mo na, maayos na lahat

^Better process it now while you are pregnant para pwede ka sa priority lane. Marriage certificate from NSO at iyong old UMID lang naman ang requirements to apply for a new UMID i.d. HTH!

^ thanks mga sis, aasikasuhin na lang nga namin isa2x, sa Dec nakaforced leave ako on the last week, hopefully open naman mga gov't agencies.
yan nga sabi namin ni hubby, we'll take advantage of the priority lane for pregnant, sana halata na akong buntis non, ngayon kasi maliit pa bump ko para lang laging busog  never pa ako nabigyan ng special treatment sa kahit anong pila

Question, magpapalit na ako status sa passport dun sa application form yun married name or maiden name ang ilalagay ko? Thank you.

If mag change ng civil status sa SSS need pa ba pumunta personally? o si HR na yung mag aayos together with BIR & PAGIBIG?

hi, Sis. In our company, Hr processes it all. we only fill up data/forms and provide specific requirement.

ako by january ko na aasikasuhin sis. wag ka mag alala, di ka nag-iisa. 1 yr and 3 months married here haha

^Ako bank account na lang at iyong phone and internet bill ko ang hindi pa updated pero the rest married name na.

^sis ano nga ulit mga aayusin na IDs? At lahat ba yun dinala mo yung mga forms sa respective offices nila? TIA!

^sis yannuh iyong sss UMID i.d. lang at iyong passport ko ang inasikaso ko same with voter's i.d. pero iyong philhealth and tin hr namin nag-asikaso.

May i know kung nag bigay ka ng original copy ng Marriage Contract? iadd ko din kasi yung son and husband ko as beneficiaries sa SSS. Original copy ba kailangan nila ng Birth cert?
