Change of Civil Status

Sinabihan si hubby ng hr nila na since my SSS and philhealth ko di pala ako pwede maging dependent ni hubby for both kasi member ako unless i decide to give up my membership. Kaso at this point am not sure whether i will work again after i give birth or not plus my premiums are not updated.

So i have a few questions sana may makatulong:

1. From covered employee need ko mag change to voluntary right?
2. Change my status (but I cant find any downloadable form online yun na ba yung mat 2?)
3. Kailangan kaya personal kong puntahan yung SSS and Philhealth for all this transactions?

Ang hirap kasi may latest ultrasound shows that i have placenta previa marginalis. And i was told to do a complete bed rest. At lalong bawal lumabas ng house.

^yes, unless you're willing to give up your membership, hindi ka talaga pwede maging dependent ng hubby mo. but this is only applicable sa philhealth. hindi ata ganon ang patakaran sa sss. ang alam ko, pwede ka dependent ng hubby mo sa sss kahit may sss kang sarili.

to answer your questions:
1. yes, if you will be a voluntary member. what i did was just go to sss, pay my contribution, and check the box that says voluntary. same goes with philhealth.
2. what status are you pertaining? yung covered employee to voluntary? or yung from single to married? if you're pertaining to change of name and status, go to sss and bring your marriage certificate. no, hindi sya ang mat-2. mat-2 is a form that you will submit to sss in claiming your maternity benefit after giving birth. same as with philhealth.
3. not sure kung pwede ipakisuyo and to just present an authorization letter and id since bedrest ka. i suggest you call their hotline to be sure.

As for status  i want to change my name and pati na rin from cover employee to voluntary. although it may be too late for me to do this and maka claim ng maternity benefits but its still worth a try.

yun din sabi ko kay hubby eh siya na lang muna tumungo doon and try to submit my docs. Padala na lang ako ng authorization letter. gusto kasi magkasama kami para sabay na date - date at kain sa labas... hehe

^as long as you haven't given birth, you can still avail your maternity benefit. pwede mo sila gawin ng sabay sabay sis.  file your mat-1, changes name, and pay as voluntary member. you can also apply for an id kung wala ka pa. pauna ka na lang sa pila if you are pregnant.

Newlywed here. 
Actually, mabilis lang makukuha ang MC copy from NSO kung kayo mismo ang maglalakad. Like kami kumuha nung MC from church at nag-file sa (LCR) Local Civil Registry's Office (same day natapos din namin, wala pa sigurong 2 hrs ang inabot, since malapit lang yung church namin sa LCR's office). Then pina-LBC lang namin yung docs to NSO and after mareceive ng NSO, punta na lang ng NSO to pay. After 10-15 working days makukuha mo na yung MC copy from NSO.

As for SSS pwedeng photocopy ng (CTC) certified true copy from LCR ang isubmit pag magche-change status. After 1 month, pwede ka na mag-apply ng ID.

Then, pag sa passport naman, pwedeng CTC with authentication from NSO ang isubmit. 

Mabilis lang naman at pwedeng ipalakad sa kamag-anak (if ever hindi convenient mag-travel), basta may authorization letter,e.  Hope this info helps.

Wala kase akong PAG-IBIG/ Philhealth, etc. kaya di ko sure kung madali lang sa mga yan.

Yung bank accounts ko, ni-leave ko na lang sa maiden name ko since ginagamit ko sya for online transactions. Open na lang ng bago under married name, which I think mas madali.

Same here, newlywed din for exactly a week now. Atat na rin ako simulan ang change name process kaya lang wala pang marriage contract. Hindi ba depende per city yung processing time nung MC sis? Yung samin kase church ang nagfile..mkelangan ba sa cityhall kami mag ask ng kahit photocopy lang or sa church talaga?

pero still yung mga legal documents ko including philhealth single pa ako
di ako makapagchange status agad2 dahil wala pa yung nso
since nasa government ako isa lang ang nag aasikaso ng change of status namin

nagpunta ako as hospital kanina
tas nag ask ako if ano isusulat ko na details ko
since yung philhealth ko ay single pa ako

advice kasi sakin ng head ko wag ko daw muna gamitin yung married surname ko
kasi di ma crecredit ng philhealth if ever

tas paglabas ng baby yung married surname na papadala mo?

Hello mga sis! I want to change my civil status sa passport ko. I set an appointment online kaso sa application form, di ko alam if yung last ko pa ba ang ilalagay ko or yung last name na ni hubby? Baka kasi hingan ako ng ID with the same last name na nakasulat sa application form eh wala pa kong ID with my husband's last name. Tingin nyo mga sis?

got the same problem din. im 33wks pregnant, then we got married last decmber lang. So problem ko din yun magpa change ng status. Aabot pa kaya kung aayusin ko sya this week? Sa mat1 ko kasi maiden name ang nagamit ko.. Tas baka magkaproblem pag iba nagamit kong name sa mat2. Pati yun sa healthcard ko sa office, ganun din. Gano ba kabilis ang change of status sa sss at philhealh?
Pwede kayang si hubby na lang ang magpalit ng status tas ako after na manganak? Thanks.

How do you change your status sa PAG-IBIG? BIR palang ang napapalitan ko e, Philhealth, SSS wala pa.

almost a year married, and i haven't legally changed my last name yet.  it costs about $500 to change my name in my immigrant visa, so noooo way that's happening

Hindi ba ot if i ask here when kayo nag start mag change ng signature? Sign ko kasi nickname + surname. Pero now that i got married parang ang weird na yung same pa rin pirma ko since my new surname na ako.

oo nga pala. for those who will be changing their names sa prc ids nila, it will take 2-3 months before the release of the id. also, kung isasabay nyo sya for the renewal, different ang payment ng renewal at ng change of name.

I renewed my license at the same time nagpapalit din ako ng surmane last Nov. 2012 pa, until now, di ko pa rin nakukuha yung prc id ko.  Sabi nila, matagal daw talaga kase dadaan pa sa legal.
Pero sobrang tagal na.

more than a year na akong married pero until now i haven't updated ibang records ko, PAGIBIG pa lang. pero i found out na pwede pa lang maupdate ang record sa philhealth online pero accidentally lang kasi i thought na wala pang philhealth number si hubby so i enrolled him online. i attached a copy of his birth certificate and our marriage certificate. then few days after, nag email ang philhealth na enrolled na raw si hubby dati pa and attached a copy of his account and updated na siya nakainclude na yung name ko.

^hindi. kung san ka unang una nag-apply, dun ka din magpapachange ng status. hassle noh?

^ not necessarily na kung saan nag-apply, sis... basta dun sa mga lto offices talaga na pinagkukuhanan ng license... yung sa malls kasi, pang-renewal lang yun... walang change of data...
