Wala pakong UMID sa SS, may philhealth ID [textspeak!] ako. Magagamit ko pba yung 2 CF1 forms ko at cert of monthly contributions from Philhealth from my employer o I need to have another forms from Philhealth? Tapos binalik nila sakin yung MAT 1 ko with stamp, tsaka anong mga month kailangan kong bayaran from SS and Phl kasi kaka resign ko nga lang, pero Im willing to go to their offices siguro by monday para magbayad. But I still need to get some infos, ano pa mga kailangan ko gawin. Thank you po sa mga sasagot
