hi mga sis,

anyone here na naka apply na ng ID na ito?

what are the qualifications?

pag single parent ka ba qualified kumuha nito?

kasi nakasulat solo parent is considered pag namatay ang spouse, or abandoned

they didn't mention about being a single parent

^Yes sis as long as di ka married, they will just ask for the birth certificate, tsaka certification from your barangay. Tanong ka sis sa city hall nyo. Di naman kelangan na patay na spouse or abandoned. Kumuha ako nito dati kasi isa sa benefits for solo parent is either they will give you 5k cash or additiona 7 days leave, annually to :D HTH :D

napanood ko kasi sa tv na may 15% discount sa gatas, clothes etc pag solo parent ka..addtional benefits ata..

so i want to apply...ok lang ba iyon kaht na parehas surname anak mo at ang tatay? di naman kami kasal

^yes sis daughter ko same last name as her MIA father, may maikling interview lang din sa dswd, dun din sa city hall namin. Madali lang sis kumuha :D

^Aw dapat di married? :( 2 yrs na kasi kaming hiwalay and recently ko lang din nalaman tong Act na to. huhuhu and kakalipat ko lang din mag 1month palang ako sa nilipatan ko.

Ask ko alng, kasi I am planning din, so need ba ang barangay clearance din? saka lahat ba ng city hall merong dswd? sa mandaluyong kasi kami eh

i cant remember sis if it was barangay clearance lang or certification from the barangay na single parent ka kasi 2008 pa ata ako last na kumuha nun. Basta punta ka sa City Hall niyo sis then ask them kung ano requirements to get a solo parent id. Alam ko meron dswd office for every city hall eh.

Hi mga sissies

UPDATE!

I asked the procedure kung paano apply...

1) need kumuha ng Barangay clearance then cedula sa baranggay niyo
2) go to the CSWD (municipyo) then they will need to ask you to have an affidavit na solo parent ka (which is in valenzuela meron narin sa municipyo)
3) needs to xerox the affidavit then needs to have a valid id (govt' issud)
4) interview
5) 1x1 pic

and yung sa affidavit na sinabi mo sis Princess_Chill, yung CSWD ba magbibigay ng form na yun for the affidavit?

I just called DSWD in our area and here are the requirements they gave me to complete (if employed):
1. Cert. of employment
2. ITR ( IF cert. of employment has compensation na, then no need for this)
3. Photocopy of marriage contract
4. Birth cert. of child(ren)
5. Affidavit of Solo Parent (I can get this sa office beside the city hall, costs P100+ lang)
6. 2pcs 1x1 ID picture

I just need to fill out a form in DSWD and submit these requirements and I can get the ID same day :)
Validity is 1yr. If you need to renew it, just give the expired ID and they will issue a new one without going through the process above na :)
