Sis, check mo sa website nila mga branches para di ka mahirapan. :)

Para sa akin, mas madali asikasuhin ang philhealth kesa sss.. And mas madali makaclaim sa philhealth. Hehe

I resigned from work last May 31.  I have been a member of Philhealth eversince I started working, 2003.  EDD ko would be on Oct.  I asked my husband na i-under na lang nya ako as dependent para hindi ko na asikasuhin yung paghulog sa philhealth ko.  I will be able to use this pa rin di ba during my delivery kahet dependent na lang ako ni hubby?

Yes sis, kaya lang mas ok kung maghulog ka na din as individual paying, lalo na kung may plan ka pa din magwork after.

In my case, i am also a dependent ni hubby, pinapatigil na nya ako maghulog, kaya lang sabi ko sayang naman kasi naghulog na ako before, kaya tinuloy ko na lang. Ngayon na preggy ako ang gagamitin kong philhealth ay yung sa akin, and yung kay hubby naman gagamitin namin philhealth for baby pag nanganak na ako. :)

^thank sis..  Glad to know na ok na pala ito.  Yes, I plan to resume my philhealth pero later na lang.  I am not too sure kase kung mag wowork pa rin ako or hindi...

hi mga sis nagkaproblem kami ngayon sa philhealth.
Last contri ko kasi sept 2010 pa. tapos nag start uli ako ng contri april 2011 lang kasi di ko na nahabol yun mga old months. manganganak ako sept 2011..

Tapos nagpreterm ako last sat 2 days ako sa ospital then syempre magfile ako sana ng philhealth. nangyari hindi daw ako pwede magclaim kasi kailangan me 12 months contri daw ako. which is june ngayon dapat me contri ako ng oct 2010 to march 2011 which is wala nga kaya ending wala hindi ako pwede magclaim.

Tapos hindi rin pwede na me philhealth ka tapos magdedependent ka ke hubby.
kaya balak namin para mahabol yun dependency ipapacancel na namin yun buong phil health ko. magrereapply nalang ako ng bagong philhealth pag nagkawork ako uli.

sayang talaga.. eto pa masaya sa kanila i paid last 2009 whole yr tapos nagclaim pa nga ako twice ng sickness at maternity. then ngayon sinasabi nila wala daw ako hulog ng 2009 ayos nga sila e sa main pa kami nagbayad nun ha..sa main din nagsabi wala nga hulog ng 2009..

Bakit ganun, nag iba ba records nila and hindi makita yung payments mo from 2009? O baka shunga shunga lang yung nakausap mo.
Sa website naman nila nakalagay 9months lang kung maternity benefits diba, and kung may sakit, 3months.

 Parang ganoon nga yung new policy nila. Parang narinig ko sa news kelan lang kailangan 12 months na ang contribution bago pwede mag claim. Im not sure though... Updated ako magbayad yearly ang payment ko every january talaga. 1200 per year, ganon din hassle pa kasi kung quarterly, mamaya makalimutan ko pa. Malaking tulong din kasi.

Nanganak ako last May 29 via CS at nagamit ko naman philhealth ko, required sakin na may atleast 9months contribution. Hiningan ako ng CF1 at CF2 na mga forms tapos receipt na from Sept2010-May2011 (9mos na bayad, gave birth ng May) kasi individually paying member ako. Automatic deduct na, sa CS ko 11k nabawas, 10k sakin tapos 1k kay baby. Separate ang CF1 at CF2 ng mommy at baby so 2 forms ang ppresent niyo, sa CF1 at CF2 ng mommy e anes at OB ang nakapirma tapos kay baby e pedia naman. Yung pre natal receipts (meds/labs/pre natal check up receipts) ko inattach ko sa CF1 at CF2 para mareimburse yung 1.5k, papadala nalang daw sakin ng philhealth yun. Sabi ng OB ko if hindi daw auto deduct ang gagawin (rereimburse nalang) mas malaki daw makukuha ko kaso ma-asikaso baka maabala daw ako kaya auto deduct nalang :)

Sa akin naman sis hindi hinahanap marriage cert. Pero palagi ako may dalang copy. Siguro kung sakaling dependent ka ni hubby baka kailangan yun. :)

hindi sis.. kailangan gamitin mo lang yun sayo.. natanung na namin yan kasi me pending reimbursement ako nito last na hospitalization ko.. inaayos nalang namin na ipacancel yun sakin kasi hindi ako pasok sa 9 months na contri. para magamit ko yun ke hubby as his dependent.

Kase, I resigned from work, syempre, mandatory naman yung pagiging philhealth member ko di ba?  Nagpapadependent ako under hubby right now, in that case, kailangan pala icancel yung philhealth membership ko?

eto yung list na nakuha ko sa hospital.. :)

1. completely  filled up philhealth form 1 na may original signature ng employer and ng miyembro ng philhealth (eto yung cf1)

- nakukuha ito galing sa employer
- para sa mga buntis na manganganak: (2) original na philhealth form 1 and kailangan
- for sugical or med treatment: (1) original philhealth form 1

2. Latest member's data record (MDR)

- malinaw na photocopy lang ang kailangan
- nakukuha ito galing sa inyong employer or philhealth office

3. malinaw na photocopy ng certified marriage certificate with registry number kung kayo ay kasal

4. kung ang pasyente ay isang dependent ng philhealth member, kailangan magsubmit ng proof of dependency na nagpapatunay dito. alinman sa mga sumusunod ay pwede ninyong i-submit (original or clear copy)

- birth certificate ng pasyente with registry number
- marriage certificare with registry number
- senior citizen id card

5. proof of philhealth contribution

employed philhealth member:
- kailangan atleast 6 months latest contribution bago ma ospital
- kailangan sa ika 10th day of every month ay nabayaran ng employer

ofw:
- kailangan ng original receipts/mdr na nakasaad ang (1) year validity nito

indigent philhealth member:
-kailangan ang original philhealth indigent card na may larawan ng miyembro at (1) malinaw na photocopy nito

individual philhealth member:
- kailangan ng latest original philhealth id card na may larawan ng miyembro at (1) malinaw na photocopy nito
- kailangan ng malinaw na latest 9 months kontribusyon bago ma-ospital
- kailangan nabayaran bago dumating ang susunod na quarter
(hindi pinapayagang mahuli sa pagbayad)
(e.g. on or before mar 31, june 30, sept 30 at dec 31 sa kasalukuyang taon)

super happy ako sa philhealth...17k ang nabawas sa akin...since hindi kami sabay umuwi ni baby, may bawas din sa bill nya, which we will know tomorrow pa. sana malaki din.

wala bang site si Philhealth na puede mo macompute kung magkano makukuha mo (like SSS?)

sabi kasi ni OB around 2k lang nababawas pag NSD :( Malaki daw pag CS pero siyempre malaki din babayaran mo.
